Health Care Open Enrollment
November 15 – Monday, December 16, 2019 at 4:30 PM
Dear KPBSD staff,
This is important: by Monday, December 16, 2019, at 4:30 PM, you will need to make a decision between two options in the High Deductible Health Plan (HDHP): the current HRA, or new HSA option. The Traditional Plan is no longer available beginning January 1, 2020.
Are you currently in the High Deductible Health Plan (HDHP)?
You have two choices within the HDHP: the current HRA, or the new HSA.
- If you want to stay in the HRA – it is what you have now – no action is required from you unless you are adding a new dependent. Your current enrollment in the HRA Plan will remain the same.
- If you choose the HSA option*: determine your eligibility*, then complete the Rehn enrollment form and pages 1, 2, and 6 of the HSA enrollment form. Make sure to return it to Stacey Cockroft no later than 4:30 on Monday, December 16, 2019.
Are you currently in the Traditional Plan?
Action is required from you: You must make a plan selection during this time because the Traditional Plan terminates on 12/31/2019 per your new Collective Bargaining Agreement. A new enrollment form is required for stop loss purposes, and you have a choice of either the HRA or the new HSA option*
within the High Deductible Health Plan (HDHP).
*Are you interested and eligible for the new High Deductible Health Plan HSA option?
If you are interested in the HSA, first read the HSA eligibility requirements prior to making your selection. If you are unsure about your HSA eligibility, you must consult a tax professional because KPBSD cannot advise you about your HSA eligibility due to the complexity of the IRS regulations. If you determine you are eligible for the HSA, complete the Rehn enrollment form and pages 1, 2, and 6 of the HSA enrollment form. Make sure to return it to Stacey Cockroft no later than 4:30 on Monday, December 16, 2019.

If you are adding a NEW dependent, legal documentation is required for that dependent.
- Spouse: Marriage Certificate (photocopy)
- Children: A photocopy of the child’s birth certificate, naming you or your spouse as the child’s parent; or appropriate court order or adoption decree naming you or your spouse as the child’s legal guardian; or a valid court order showing that the employee must provide health insurance.

All legal documents and other required documents must be turned in to Stacey Cockroft at the district office by the deadline.
